Real-World Use of Highly Sensitive Liquid Biopsy Monitoring in Metastatic Breast Cancer Patients Treated with Endocrine Agents after Exposure to Aromatase Inhibitors

چکیده

Endocrine-resistant, hormone receptor-positive, and HER2-negative (HR+/HER2-) metastatic breast cancer (mBC) is largely governed by acquired mutations in the estrogen receptor, which promote ligand-independent activation, truncal alterations PI3K signaling pathway, with a broader range of gene occurring less prevalence. Circulating tumor DNA (ctDNA)-based technologies are progressively permeating clinical setting. However, their utility for serial monitoring has been hindered significant costs, inter-technique variability, real-world patient heterogeneity. We interrogated longitudinal collection 180 plasma samples from 75 HR+/HER2- mBC patients who progressed or relapsed after exposure to aromatase inhibitors were subsequently treated endocrine therapy (ET) means highly sensitive affordable digital PCR SafeSEQ sequencing. Baseline PIK3CA TP53 prognostic shorter progression-free survival our population. Mutant was subset receiving fulvestrant monotherapy progression CDK4/6 inhibitor (CDK4/6i)-containing regimen, its suppression predictive case long-term benefit alpelisib. ESR1 did not receive concurrent CDK4/6i, an impact influenced variant allele frequency, early strongly efficacy associated whole cohort. Mutations ESR1, TP53, KRAS emerged as putative drivers resistance. These findings collectively contribute characterization ctDNA cases previously exposed support ongoing studies either targeting actionable leveraging ultra-sensitive tracking ctDNA.

Efficacy of Exemestane in Korean Patients with Metastatic Breast Cancer after Failure of Nonsteroidal Aromatase Inhibitors

PURPOSE Exemestane has shown good efficacy and tolerability in postmenopausal women with hormone receptor-positive metastatic breast cancer. However, clinical outcomes in Korean patients have not yet been reported. METHODS Data on 112 postmenopausal women with metastatic breast cancer were obtained retrospectively. Clinicopathological characteristics and treatment history were extracted from ...

Role of combination therapy with aromatase and cyclooxygenase-2 inhibitors in patients with metastatic breast cancer.

Aromatase inhibitors (AIs) are well established in the treatment of metastatic hormone-sensitive breast cancer in postmenopausal women. Cyclooxygenase (COX)-2 inhibitors have demonstrated efficacy in reducing cancer risk in animal and human studies. In several preclinical studies, combination AI plus COX-2 inhibitor therapy has shown a synergistic antitumor effect. This review describes the uti...

Efficacy of exemestane after nonsteroidal aromatase inhibitor use in metastatic breast cancer patients.

BACKGROUND Previous studies have suggested a lack of complete cross-resistance between steroidal (exemestane) and non-steroidal aromatase inhibitors (nSAI). METHODS Eighty-eight metastatic breast cancer (MBC) patients who received 25 mg of exemestane orally once a day at the National Cancer Center, Korea, between 2003 and 2009, were reviewed retrospectively. All patients had received nSAI for...

Clinical validation of prospective liquid biopsy monitoring in patients with wild-type RAS metastatic colorectal cancer treated with FOLFIRI-cetuximab

Cancer genomics and translational medicine rely on the molecular profiling of patient's tumor obtained during surgery or biopsy. Alternatively, blood is a less invasive source of tumor DNA shed, amongst other ways, as cell-free DNA (cfDNA). Highly-sensitive assays capable to detect cancer genetic events from patient's blood plasma became popularly known as liquid biopsy (LqB). Importantly, retr...

Healthcare Resource Use and Expenditures among Metastatic Breast Cancer Patients Treated with HER2-Targeted Agents

Objective. To compare healthcare utilization (HCU) and costs of women newly diagnosed with metastatic breast cancer (mBC) by receipt of HER2-targeted agents (H2T) and among H2T subgroups. Methods. Adult women newly diagnosed with mBC (index date) during 2008-2012 were followed until enrollment end or inpatient death. Study cohorts were antineoplastic ± H2Ts, and no treatment; and subgroups of H...
